It has to be either a wiring problem (check connector to the MAF and wiring) or a bad component (they can be bad out out of the box, it does happen)-the MAF cannot have any leaks before or after it to read properly, and the sensor wire is very sensitive to cleaners and should not be handled. Your miss at cylinder three may or may not be related, but try swapping plug wires, coils, or injectors to see if the problem moves. The only other thing is the PCM-check the connector and the pins (look for a bent pin), consider getting another PCM and throwing it in to see if it cures it. The dealer blaming it on 'ghosts' is a stupid and lame excuse for not wanting to solve this problem, there is a rational reason for this happening.
